SUBJECT:                     Amendments to Master Agreements for Substance Use Disorder Residential Services

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute Amendment II to Master Agreement No. 18-692 with Non-Drug Medi-Cal Residential Treatment Service vendors to reallocate funds and update insurance language, effective upon execution with no change in term of January 1, 2019 to June 30, 2022 and decreasing the maximum by $1,500,000 to a total of $13,012,096; and,

 

2.                     Approve and authorize the Chairman to execute Amendment I to Master Agreement No. 18-693 with Non-Drug Medi-Cal Residential Room and Board vendors, to increase substance use disorder treatment services and update insurance language, effective upon execution with no change in term of January 1, 2019 to June 30, 2022 and increasing the maximum amount by $1,500,000 to a total of $14,007,418.

REPORT

Approval of the recommended actions will reallocate FY 2021-22 funds from the Master Agreement for Non-Drug Medi-Cal Residential to the Master Agreement for Non-Drug Medi-Cal Residential Room and Board to cover program costs not reimbursed by other funding sources.  Services are funded with Substance Abuse Block Grant (SABG) and Realignment, with no increase to Net County Cost.  This item is countywide.

 

ALTERNATIVE ACTION(S):

 

If the recommended actions are not approved, the Department of Behavioral Health would not be able to fund the room and board costs associated with residential and withdrawal management substance abuse disorder treatment services.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended actions. The recommended actions will reallocate $1,500,000 from the Residential Treatment Service to the Residential Room and Board master agreement.  The agreements for Non-Drug Medi-Cal Residential Treatment Service ($13,012,096) and Non-Drug Medi-Cal Residential Room and Board ($14,007,418) are funded by SABG and Behavioral Health Realignment.  Sufficient appropriations and estimated revenues are included in the Department’s Org 5630 FY 2020-21 Adopted Budget and will be included in future budget requests.

 

 

 

DISCUSSION:

 

On December 4, 2018, the Board approved State Agreement No. 18-95145 an intergovernmental agreement with the Department of Health Care Services (DHCS) to provide substance use disorder treatment services under a new service delivery model known as the Drug Medi-Cal Organized Delivery System (DMC-ODS). The DMC-ODS is a pilot program designed to test the effectiveness of an organized delivery of treatment services for Medicaid-eligible persons served.  Under the DMC-ODS, counties are required to ensure an adequate network of services are available for the Medi-Cal population. 

 

On December 11, 2018, the Board approved Master Agreement No. 18-692 with Non-Drug Medi-Cal Residential Treatment Service vendors to provide adult perinatal and non-perinatal residential substance use disorder treatment services.  On the same date, the Board approved Master Agreement No. 18-693 with Non-DMC Residential Room and Board vendors to provide room and board for persons receiving residential, residential perinatal, and/or withdrawal management substance use disorder treatment services. 

 

Fresno County’s DMC-ODS services became operational on January 1, 2019. 

 

On January 29, 2019, the Board approved an amendment to Master Agreement No. 18-692 to waive DMC certification requirements for a 12-month term beginning January 1, 2019 to allow providers to deliver reimbursable treatment services while continuing to pursue DMC certification through DHCS.  

 

The recommended amendments would reallocate $1,500,000 between Master Agreement Nos. 18-692 and 18-693 for the 12-month term of July 1, 2021 through June 30, 2022 to continue funding essential residential perinatal, residential non-perinatal and/ or withdrawal management treatment and room and board services.  The amendments also update insurance requirements to meet current County standards.  All other terms remain unchanged.
