REPORT
There is no increase in Net County Cost associated with the recommended actions. Approval of the first recommended action will retroactively authorize the submission of a grant application to the Kaiser Foundation Hospitals for funding under the Expanding Fresno County Mobile Health Safety-Net Grant Program. It should be noted the Department of Public Health (Department) was invited by Kaiser Permanente Fresno to submit a grant proposal for funding by no later than July 8, 2026. To meet the application deadline, the Department submitted the application on July 2, 2026, contingent upon the Board’s approval. Kaiser approved the application and sent the agreement to the Department on August 25, 2026. Approval of the second recommended action will provide funds and support to the Department Mobile Health (MH) Program. Approval of the third and fourth recommended actions will adjust the Department of Public Health Org 5620 and Special Revenue Fund (SRF) Miscellaneous Public Health Org 1166 appropriations and revenues by $76,358 each. Approval of the fifth recommended action will authorize the Department of Public Health Director or his designee to execute any documents or release forms needed to fulfill the grant. This item is countywide.
ALTERNATIVE ACTION(s):
There is no viable alternative action. If your Board chooses not to approve the recommended actions, the Department will withdraw the submitted grant application from consideration and will not receive additional funding to enhance its capacity to conduct health events for the Department's MH Program.
RETROACTIVE AGREEMENT:
The agreement with Kaiser Foundation Hospitals is retroactive to October 1, 2026, due to the Department receiving the grant agreement from Kaiser Foundation Hospitals on August 25, 2026, which did not allow enough time for it to be properly processed and brought to your Board prior to the October 1, 2026 start date. There are currently no outstanding invoices related to this funding, and it is being brought to your Board in accordance with agenda item processing timelines.
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended actions. If approved, the grant will provide $76,358 in one-time funding for October 1, 2026 to September 30, 2027. Approval of the third recommended action will increase the Department of Public Health Org 5620 FY 2026-27 appropriations and revenues by $76,358 for the additional costs associated with MH Program. The revenues will be reflected via an Operating Transfer Out from the SRF Miscellaneous Public Health Org 1166. The fourth recommended action will increase SRF Org 1166 appropriations and revenues by $76,358 to account for the transfer of funds.
DISCUSSION:
Approval of the recommended grant application and agreement will provide the Department with additional funding to support and expand the MH Program so agricultural workers and other underserved Fresno County residents can access no-cost clinical health services. These funds are estimated to support up to 16 additional mobile health events delivered by the current MH subcontractors Saint Agnes Medical Center and University of California, San Francisco, and any other providers subcontracted within the respective agreement period.
Adjustments to the Departments various funds are also needed to update the FY 2026-27 Adopted Budget. These budget adjustments are due to the grant application opening after all budget preparations had been finalized and will ensure the Department is able to fully and properly utilize the funds.
The recommended agreement includes mutual indemnification. The Department has determined that accepting the indemnification language is a prudent business decision, as the Kaiser grant funds the Department’s MH Program and will help the Department achieve its goals of providing no-cost health events to rural and agricultural residents of the County of Fresno.
