RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute an Agreement with Turning Point of Central California, Inc. for the provision of Mental Health Services Act (MHSA) Full Service Partnership Services, Outpatient Mental Health Services, and Intensive Case Management Services in rural Fresno County for children that are severely emotionally disturbed and adults that are seriously mentally ill, effective July 1, 2018, not to exceed five consecutive years, which includes a three-year base contract and two optional one-year extensions, total not to exceed $63,403,585

REPORT

Approval of the recommended action will allow for the operation of Full Service Partnership, Outpatient, and Intensive Case Management mental health services in rural Fresno County for a three-year base contract and two optional one-year service extensions.  Turning Point of Central California, Inc. (Turning Point) was selected following a Request for Proposal (RFP) process.  Turning Point is currently the provider of rural mental health services and will continue to provide services on July 1, 2018 with no increase in Net County Cost.  The services will continue to be provided in Coalinga, Kerman, Pinedale, Reedley, Sanger, and Selma, while additional sites have been identified for expansion in Huron and Mendota.  The services will be funded with a combination of client fees, Mental Health Services Act and Short/Doyle Medi-Cal Federal Financial Participation funds.

 

ALTERNATIVE ACTION(S):

 

There are no viable alternative actions.  If the recommended action is not approved, the Department of Behavioral Health would be unable to provide mental health services to rural Fresno County residents.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action.  The maximum cost of the recommended agreement is as follows:

 

                     Full Service Partnership for $10,708,965

                     Outpatient Mental Health and Intensive Case Management for $52,694,620

 

The recommended agreement has provisions for a three-year base contract and two optional one-year extensions through June 30, 2023.  The maximum compensation for the term is $63,403,585.  The annual compensation will be as follows:

 

                     FY 2018-2019: $11,696,525

                     FY 2019-2020: $12,202,948

                     FY 2020-2021: $12,699,218

                     FY 2021-2022: $13,167,467

                     FY 2022-2023: $13,637,427

 

Sufficient appropriations and estimated revenues will be included in the Behavioral Health Org 5630 FY 2018-19 Recommended Budget and will be included in subsequent budget requests for the duration of the contract term.

 

DISCUSSION:

 

On September 23, 2008, your Board approved the Agreement with Turning Point of Central California, Inc. for the provision of rural mental health services in Pinedale, Sanger and Reedley, including FSP, ICM, and OP Programs.  On June 18, 2013, your Board once again approved the Agreement with Turning Point to continue providing rural mental health services.  Rural services were established to serve adults and older adults with serious mental illness who were homeless or at risk of homelessness, frequent users of crisis services, and/or incarcerated.  The FSP program currently serves a minimum of 175 clients annually.  The OP and ICM programs currently serve a minimum of 2,400 clients annually.  The Rural programs utilize a welcoming, recovery oriented, integrated, service delivery model that uses innovative interventions to reduce crisis services, admissions into inpatient facilities, or jail.  The current Agreement expires on June 30, 2018.

 

On March 14, 2018, RFP No. 18-034 For Rural Mental Health services was released by the ISD - Purchasing Division (ISD - Purchasing).  The RFP sought bidders that would provide FSP, OP, and ICM services to rural Fresno County residents.  Addendum No. 1 was issued on March 28, 2018 following a Bidders’ Conference on March 23, 2018.  The RFP closed on April 12, 2018.

 

A total of one proposal was received and evaluated according to ISD - Purchasing guidelines based on a combination of proposal content, quality of proposed services, and experience in delivering treatment services to the target population.  The review committee consisting of one staff analyst and one community mental health specialist from the Department of Behavioral Health, and one member of the Fresno County Behavioral Health Board, was able to reach a consensus and develop a recommendation that would best meet the critical needs of the County’s rural population.

 

Approval of this Agreement will allow Turning Point to continue providing uninterrupted rural services in the six existing locations - Coalinga, Kerman, Pinedale, Reedley, Sanger, and Selma.  In addition, approval of this Agreement will allow Turning Point to expand rural services into two new locations - Huron and Mendota.  Turning Point has identified these two locations as an avenue to provide easier access to services, further increase penetration rates in rural areas, and reduce wait times in nearby communities such as Coalinga and Kerman.

 

The recommended agreement includes language for termination by Turning Point and/or the County, or their designees, upon a 60-day advanced written notice and allows for the termination due to non-allocation of funds upon the giving of a 30-day advanced written notice to the contractor. The agreement also includes language for modifications to the budget, volume of service units provided, and electronic health records charges, which may be made with written approval of the County’s DBH Director or designee.
