DATE: October 20, 2026
TO: Board of Supervisors
SUBMITTED BY: Joe Prado, Director, Department of Public Health
SUBJECT: Amendment No. 3 to Agreement with Fresno Economic Opportunities Commission
RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute Amendment No. 3 to Agreement No. A-25-467 with Fresno Economic Opportunities Commission to update the agreement’s FY 2026-27 expenditure limit budget, effective upon execution, with no change to the term of July 1, 2023, through June 30, 2027, or compensation maximum of $1,100,000.
REPORT
There is no additional Net County Cost associated with the recommended action. The action will allow Fresno Economic Opportunities Commission (Fresno EOC) to revise its Fiscal Year (FY) 2026-27 budget to maximize available funding to ensure completion of all school-based sealant program activities through preventative population health intervention care-coordination services. This item is countywide.
ALTERNATIVE ACTION(S):
Should your Board choose not to execute the recommended Amendment, Fresno EOC would not be able to complete the scheduled activities related to population health intervention services for students and their families who are receiving oral health preventative screenings through Fresno EOC’s school-based dental sealant program in elementary schools. Not implementing all the care coordination activities in the targeted populations could result in a negative effect on Fresno EOC’s programmatic efforts to create a sustainable school-based sealant program in future years.
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended action. The recommended Amendment is fully funded with Oral Health grant funds and Population Health Management grant funds ($1,100,000). Sufficient appropriations and estimated revenues are included in the Department’s Org 5620 FY 2026-27 Adopted Budget.
DISCUSSION:
On June 6, 2023, your Board approved an Amendment to increase the oral health grant allocation from the California Department of Public Health to allow the Local Oral Health Program (LOHP) to continue the school-based/school-linked activities not completed
in the previous grant term.
On August 8, 2023, your Board approved Agreement No. A-23-396 with Fresno EOC to leverage the reallocated funding to complete unfinished school-based/school-linked activities from the previous oral health grant, continue providing their school-based dental sealant program in elementary schools, and assist elementary schools with the mandated state oral health reporting requirements.
On October 22, 2024, your Board approved First Amendment Agreement No. 24-556 with Fresno EOC to increase the maximum compensation to allow Fresno EOC to continue their school-based dental sealant program and the distribution of dental stipends to the dentists and dental hygienists who participate in the school-based oral health screening events during FY 2023-24, and expand their program to include a sealant retention check on students who receive dental sealants or fluoride varnishes during the school year.
On September 9, 2025, your Board approved Second Amendment Agreement No. 25-467 with Fresno EOC to increase the maximum compensation to allow Fresno EOC to expand their program to include Population Health Management care-coordination activities, including screening for Social Determinants of Health for urgent referrals, increasing collaboration with the Fresno County Community Health Worker Network to document and track outcomes for children and families and to develop referral criteria and the protocol for linking children to a source of dental care. Funding for dental stipends through FY 2026-27 was approved and to date, Fresno EOC has been able to reach a total of 1,929 elementary school-aged students to provide dental sealants, dental cleaning, or a fluoride varnish treatment.
If approved, the recommended action will allow Fresno EOC to consolidate personnel positions, update position responsibilities, modify the operating expenses, travel expenses, and other expense budget categories in FY 2026-27 to focus their collaborative efforts on completing care-coordination activities by December 31, 2026, and to align with the end date for Population Health Management funds. Completion of the remaining programmatic activities and the programs last day under agreement with the County’s LOHP is June 30, 2027. Upon sunsetting of the current agreement, Fresno EOC’s goal is to continue reaching school-aged children to provide preventative oral health services through a long-term sustainable school-based sealant program.
