DATE: September 10, 2024
TO: Board of Supervisors
SUBMITTED BY: David Luchini, RN, PHN, Director, Department of Public Health
SUBJECT: Second Amendment to Agreement with Care Coordination Systems, LLC
RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute Second Amendment to Agreement with Care Coordination Systems, LLC for upgrades within the integrated data sharing platform and resource integration enhancements for the Department of Public Health’s community health worker programs, effective upon execution with no change to the term of May 17, 2022 through May 16, 2027 and increasing the maximum by $140,000 to a total of $1,309,250.
REPORT
There is no additional Net County Cost associated with the recommended action. The recommended action will allow for Care Coordination Systems, LLC (CCS) to build and implement platform updates to the Department of Public Health’s (DPH) data sharing platform for the community health worker (CHW) network. Updates include accessibility improvements consistent with the Americans with Disabilities Act (ADA), 211 resource integration to connect shared resources and referrals between CCS and 211, and integration of the community information exchange (CIE) to create bidirectional exchanges of shared client information between CCS and the CIE. The recommended action will also increase CCS’s total compensation for the term by $140,000 to pay for the cost of these services. The recommended action is fully funded with existing grant funds. This item is countywide.
ALTERNATIVE ACTION(S):
Should your Board not approve the recommended action, CCS will not be able to implement changes to the data platform and necessary functions will not be available to the County. Without ADA improvements, County staff and subcontracted partners with visual or hearing impairments will not be able to effectively use CCS. Without 211 and CIE integration CCS will not be able to easily communicate with other data sharing platforms and information will not be able to be shared.
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended action. The maximum compensation of the agreement with CCS will increase by $140,000 to $1,309,250 for the contract term. The recommended action is fully funded by federal grant revenues including The Centers for Disease Control and Prevention’s Initiative to Address COVID-19 Related Health Disparities (Health Disparities) and Epidemiology and Laboratory Capacity COVID-19 (ELC) funds. Sufficient appropriations and estimated revenues are included in DPH’s Org 5620 FY 2024-25 Recommended Budget and will be included in subsequent budget requests.
DISCUSSION:
On May 17, 2022, your Board approved Agreement No. 22-214 which allowed the County to contract with CCS to provide an integrated data sharing platform, user licenses, and trainings to County staff as well as subcontracted partners. Since the agreement’s execution, CCS has acted as a referral source and data sharing platform for multiple County programs currently implementing CHW network activities. On March 28, 2023, your Board approved Agreement No. 23-127 which amended Agreement No. 22-214 to allow CCS to provide 60 additional user licenses and trainings as well as create custom data mapping and collection forms.
Since execution, various County and subcontracted partner staff have utilized the CCS platform. As a partnering agency, Valley Center for the Blind was able to identify limitations with CCS’s current user interface and enhancements needed to be operational in accordance with ADA standards. Addressing these changes will allow all users to effectively utilize the CCS platform to perform necessary job functions. As the County strives for equitable practices through service delivery, DPH is committed to ensuring that necessary tools and resources for staff and clients are accessible to the highest standards possible.
CCS has the ability to interface with CIE to provide bidirectional referrals and exchange shared client information, allowing for better data and service integration. Fresno County’s CIE is currently being designed and implementation is projected to be in early 2026. CCS will need to start working on data integration plans in 2025 to ensure compliance with the 2026 CIE launch date.
On October 25, 2022, your Board approved Agreement No. 22-482 which allowed the County to contract with United Way Fresno and Madera Counties to provide access to their database of resources, 211 services, perform targeted outreach to focused areas of Fresno County. The recommended action will allow for 211 resource integration into CCS for CHW access to these resources and direct linkage of services through the CCS platform. The recommended action will also allow for necessary ADA improvements and CIE integration into the CCS platform. Finally, the recommended action will allow an increase of the maximum compensation by $140,000 to build out, implement, maintain, and monitor these additions.
