SUBJECT: Retroactive Grant Application and Award with the California Department of Health Care Services
RECOMMENDED ACTION(S):
TITLE
1. Retroactively approve and authorize the Probation Department’s previous submittal of a grant application to the California Department of Health Care Services for Providing Access and Transforming Health Justice-Involved Capacity Building Program Round 4 Grant related to the implementation of the Statewide California Advancing and Innovating Medi-Cal Initiative; and
2. Receive award and retroactively authorize the Chief Probation Officer or designee to electronically sign and submit the Acknowledgement of Grant Terms and Conditions form and execute any documents necessary with the Department of Health Care Services to accept and collect the grant in the amount of $300,000.
REPORT
Approval of the recommended actions will authorize the retroactive submission of a grant application to the California Department of Health Care Services (DHCS) through the California Advancing & Innovating Medi-Cal (CalAIM) Initiative for the Providing Access and Transforming Health (PATH) Justice-Involved (JI) Round 4. The funding provides Round 3 awardees with additional funding to support the planning and implementation of targeted pre-release Medi-Cal services to eligible individuals in correctional facilities, as outlined in the CalAIM Section 1115 Demonstration approval.
It should be noted that DHCS published a request for applications on May 12, 2025. In order to meet the application deadline of July 11, 2025, the Probation Department (Probation) submitted the application on July 9, 2025, contingent upon the Board’s approval. DHCS issued the award notification on April 21, 2026, along with the Grant Terms and Conditions, which serve as the grant agreement. This item is countywide.
ALTERNATIVE ACTION(S):
There is no alternative action. If your Board does not approve the recommended actions, the submitted grant application will be withdrawn from consideration for funding, resulting in loss of additional funding to invest in staffing, collaborative planning and implementation of targeted pre-released Medi-Cal services to youth at the Juvenile Justice Campus (JJC).
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended actions. If accepted, the grant will provide Probation up to $300,000 in accordance with the DHCS PATH JI 3/4 Awardee Guidance May 2026. A final progress report and budget will be submitted to DHCS on July 15, 2026. There is no match requirement, and sufficient appropriations and estimated revenues will be included in Probation’s FY 2026-27 Recommended Budget for Org 3440 and in future budget requests.
DISCUSSION:
California is the first state in the nation to offer a targeted set of Medi-Cal services to individuals in state prisons, county jails, and youth detention facilities for up to 90 days before release. Currently, Medi-Cal services are generally available only after release. Through a federal Medicaid 1115 demonstration waiver, DHCS established a coordinated community reentry process to help individuals leaving incarceration connect to needed physical and behavioral health services upon release. DHCS also received federal approval under the CalAIM 1115 Demonstration for PATH capacity building funds to support the JI Reentry Initiative.
PATH Rounds 1 and 2 supported correctional agencies in collaborative planning with county social services departments to identify processes, protocols, and IT system changes needed to implement pre-release services, with a focus on establishing Medi-Cal eligibility screening and application processes. On April 11, 2023, the Board retroactively approved the Sheriff’s Office submission of the Round 1 application, originally submitted to DHCS on July 20, 2022, and authorized acceptance of the $150,000 award, allocated as $100,000 to the Sheriff’s Office and $50,000 to Probation. On October 10, 2023, the Board retroactively approved the Department of Social Services (DSS)’ Round 2 application submitted to DHCS on March 30, 2023, with award amounts of $1,002,290 for the Sheriff’s Office, $599,494 for DSS, and $73,631 for Probation.
Round 3 provided planning and implementation grants to support investments in personnel, capacity, and IT systems needed to implement pre-release service processes and establish behavioral health linkages for community reentry services as required by AB 133. On November 7, 2023, the Board retroactively approved the Round 3 grant applications previously submitted to DHCS by the Department of Behavioral Health (DBH) on July 27, 2023, and by the Sheriff’s Office and Probation on July 31, 2023. Round 3 award amounts were $5,000,000 for the Sheriff’s Office, $2,500,000 for Probation, and $1,984,121 for DBH.
Round 4 allowed Round 3 awardees to apply for additional funding to support the planning and implementation of targeted pre-release Medi-Cal services for eligible individuals in state prisons, county jails, and youth correctional facilities, as outlined in the CalAIM Section 1115 Demonstration approval. The application period opened on May 12, 2025, and closed on July 11, 2025. After discussions with stakeholders regarding additional staffing, technology, and training needs, Probation submitted an application on July 9, 2025, requesting up to $1,358,117 to supplement existing activities and fund new permissible uses not included in its Round 3 budget. Due to the volume of applications, DHCS delayed award decisions and limited funding to correctional facilities, setting award amounts based on Go-Live status and progress toward implementation. Probation received its award notification on April 21, 2026. Approval of recommended action item one will authorize Probation’s grant application to DHCS. Approval of recommended action item two will authorize Probation to sign and submit the Terms and Conditions and receive the grant funds to support implementation efforts. Probation will also submit a revised scope of work and budget as part of the Final Progress Report due July 15, 2026, to reflect the $300,000 award amount.
