SUBJECT:                     Revenue Agreement with the California Department of Public Health for the Type 2 Diabetes Prevention Program

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute a revenue agreement with the California Department of Public Health for the Type 2 Diabetes Prevention Program to address chronic disease prevention, effective upon approval of California Department of General Services through June 30, 2028, total not to exceed $237,000; and

 

2.                     Approve and authorize the Director of the Department of Public Health, or designee, to execute the Contractor’s Release for the final invoice and other forms as required by the proposed Agreement.

REPORT

There is no additional Net County Cost associated with the recommended actions. Approval of the first recommended action will allow the Department of Public Health (Department) to implement an agreement with California Department of Public Health (CDPH) to support the Department’s effort to address chronic disease by providing prevention and health equity support services to populations of Fresno County with the highest health disparities. Approval of the second recommended action will allow the Director of the Department to execute the State Contractor’s Release form required to be submitted with the final invoice to CDPH. This item is countywide.

 

ALTERNATIVE ACTION(S):

 

Should your Board not approve the recommended actions, the Department would be unable to provide type 2 diabetes intervention services to vulnerable community members.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended actions. The recommended revenue agreement is funded by the CDPH Type 2 Diabetes Program (T2DP), provided by the Centers for Disease Control and Prevention (CDC).  The maximum compensation of $237,000 ($59,250 annually) will pay for salary and benefits, operational, and indirect costs to execute services for the management of type 2 Diabetes.  Sufficient appropriations and estimated revenues will be included in the Department’s Org 5620 FY 2024-25 Recommended Budget and will be included in subsequent budget requests.

 

DISCUSSION:

 

On February 26, 2019, your Board approved acceptance of Cooperative Agreement Award No. 1 NU58DP006631-01-00 from the CDC which allowed the Department to implement health strategies to prevent and manage diabetes, heart disease, and stroke through its Collaborating for Wellness Program from September 30, 2018 through September 29, 2023. Since the conclusion of the Collaborating for Wellness Program, CDPH has offered an allocation of non-competitive funding to the Department through the T2DP Program to provide similar services.                

 

Diabetes is one of the leading causes of death in the County, approval of this agreement will allow the Department to leverage and strengthen existing partnerships to address this disease.  One of the program’s objectives is to increase the number of recognized National Diabetes Prevention Program (NDPP) providers in the County.  There are currently no NDPP providers in rural areas of the County, this funding will allow the County to partner with a current contracted local federally qualified health center to become NDPP certified and offer this program to rural residents of the County.  Funding will also allow for trainings to local community health workers and rural mobile health interventions to increase their knowledge and referral abilities to these services.

 

The T2DP Program objectives are in alignment with the Department’s commitment to advance health equity within Fresno County and serve the most vulnerable populations experiencing the highest levels of health disparities. The T2DP Program also enhances the Department’s capacity building efforts for local partners and will strengthen sustainability efforts for various local programs. If approved, the Agreement will allow the Department to implement statewide evidenced-based approaches to diabetes management and type 2 diabetes prevention.

 

Fresno County Risk Management Division has approved the agreement with the disclosure that the recommended agreement contains a one-way indemnification in favor of the State.
