SUBJECT:                     Amendments to Agreements for Substance Use Prevention Services

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute Amendment IV to Agreement No. 11-506 with California Health Collaborative, for the provision of marijuana prevention services for youth and young adults, effective upon execution through June 30, 2016 ($1,700,000)

 

2.                     Approve and authorize the Chairman to execute Amendment IV to Agreement No. 10-367 with California Health Collaborative, for the provision of prescription drug abuse prevention services for youth and young adults, effective upon execution through June 30, 2016 ($1,960,000)

 

3.                     Approve and authorize the Chairman to execute Amendment IV to Agreement No. 10-366 with Youth Leadership Institute, for the provision of alcohol use and binge drinking prevention services for youth and young adults, effective upon execution through June 30, 2016 ($2,120,000)

 

4.                     Approve and authorize the Chairman to execute Amendment III to Agreement No. 13-545 with Fresno County Office of Education, for the provision of alcohol and other drug use prevention services for youth and young adults, effective upon execution through June 30, 2016 ($1,124,653)

 

REPORT

Approval of the recommended actions will allow the Department of Behavioral Health and its current providers to continue prevention services in accordance with the County five-year Strategic Prevention Plan (SPP) for Alcohol and Other Drugs as required by the State, pursuant to the Federal Substance Abuse and Mental Health Services Administration.  The development of the 2015-2020 SPP required additional time to finalize due to data issues and revisions required by the California Department of Health Care Services (DHCS).  The recommended Amendments will not impact the level or quality of services provided under the Agreements and will allow sufficient time for the Department to release a Request for Proposals with the DHCS approved revisions to the SPP and have new providers in place by July 1, 2016.  The services are provided to youth and young adults between the ages of 12 to 20, with no net County cost. 

 

ALTERNATIVE ACTION(S):

 

If the recommended actions are not approved, there will be an interruption in alcohol and other drug use (AOD) prevention services for approximately 30,000 at-risk youth and young adults within the County.  If not approved, these critical prevention services provided by Department and its partners will be delayed until new agreements are in place.

 

 

 

FISCAL IMPACT:

 

There is no increase in net County cost associated with the recommended actions.  The annual maximum annual amount for the combined Agreements is $1,424,653, and is fully offset with Substance Abuse Prevention and Treatment (SAPT) Federal Block Grant and Behavioral Health Realignment funds.  The initial Agreements became effective on July 13, 2010.  The amount of the existing Agreements ($6,192,327) will increase $712,326, for the six month period (January 1, 2016 to June 30, 2016).  Sufficient appropriations and estimated revenues are included in the Department of Behavioral Health Org 5630 FY 2015-16 Adopted Budget.

 

DISCUSSION:

 

The Fresno County Strategic Prevention Plan was the result of a process guided by the Strategic Prevention Framework (SPF) developed by the Substance Abuse and Mental Health Administration (SAMHSA) of the US Department of Health and Human Services.  Prevention services are provided to approximately 30,000 youth and young adults ages 12 to 20.  Prevention service participants take part in programming and activities on a voluntary basis during school hours, after school and evenings and/or weekends.  Participants are referred to appropriate ancillary services on an as-needed basis. 

 

The SPP was originally approved by the State Department of Health Care Services (DHCS) in April 2015.  Approval was subsequently rescinded on June 4, 2015, following further DHCS review.  DHCS required revisions to the SPP before final approval was given on July 20, 2015. The scope of services in the Request for Proposals (RFP) could not be finalized until SPP approval was received.  A six month extension is requested for each of the existing Agreements to minimize the impact to the school-based sites due to a disruption of program services while the RFP process is completed. 

 

The RFP will be submitted to Purchasing in November 2015 to seek prevention service providers with programs and/or activities that align with the goals and outcomes specified in the new SPP.  The intent is to have new service providers in place by July 1, 2016.

 

The existing prevention service Agreements are scheduled to terminate on December 31, 2015.  Approval of the recommended actions will extend the term an additional six (6) month period through June 30, 2016, to allow for the RPF process and new contracts to be executed without an interruption to prevention services.

 

The 20% benefit maximum should not be applied to these contracts as California Health Collaborative, Fresno County Office of Education and Youth Leadership Institute do not primarily serve Fresno County.  The California Health Collaborative, Fresno County Office of Education and Youth Leadership Institute are nonprofit agencies and Fresno County is not their sole client.  The California Health Collaborative receives approximately 16% of their annual budget from Fresno County contracts for the Lock It Up Project and Performing Above the High Project.  The Fresno County Office of Education receives approximately 1% of their annual budget from Fresno County contracts.  The Youth Leadership Institute receives approximately 15% of their annual budget from Fresno County contracts.
