RECOMMENDED ACTION(S):

Approve and authorize the Chairman to execute Retroactive Amendment No. 2 to Agreement No. 23-295 with Kings View, effective November 1, 2025, with no change to the term of July 1, 2023, through June 30, 2026, rolling over $14,802 in unspent funds from FY 2024-25, and increasing the maximum compensation by $8,448 to a total of $4,169,968.
REPORT

There is no additional Net County Cost associated with the recommended action, which will increase the program budget for Projects for Assistance in Transition from Homelessness (PATH) Street Outreach Team and Rural Support (STARS) using Homeless Housing Assistance Prevention (HHAP) and roll over unspent American Rescue Plan Act (ARPA) funds from the prior fiscal year to the current fiscal year. The additional HHAP and ARPA funds will support enhanced rural outreach services under the PATH-STARS program. This item is countywide.

 

ALTERNATIVE ACTION(S):

 

If your Board does not approve the recommended action, the Department will be unable to fully expend the HHAP funds provided by the Department of Social Services (DSS) by the required deadline of December 31, 2025. Additionally, the County Administrative Office (CAO) has authorized the rollover of unspent ARPA funds from FY 2024-25 into FY 2025-26, with a final expenditure deadline of May 31, 2026. Failure to utilize these funds will result in the forfeiture of additional financial support for the Kings View (KV) PATH-STARS program, which plays a critical role in homeless outreach efforts.

RETROACTIVE AGREEMENT(S):

 

The recommended amendment will be made retroactive to November 1, 2025, to account for the use of mobile showers under the KV PATH-STARS program. This adjustment supports planned rural outreach efforts led by the PATH-STARS program staff in collaboration with the CAO.

 

The KV team has secured sites for shower operations in Sanger and Selma beginning November 6, 2025 and November 13, 2025 respectively and has support from those jurisdictions to operate the shower trailer. Services will focus on engaging individuals experiencing homelessness in underserved rural areas. These efforts will include the deployment of a mobile shower unit, distribution of hygiene kits, and connection to supportive services such as behavioral health, housing navigation, and case management. The retroactive application of the amendment ensures continuity of services and enables program staff to deliver these critical supports without interruption.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action. Annual compensation is funded with Medi-Cal Federal Financial Participation (FFP), Realignment, Mental Health Services Act (MHSA), HHAP, and ARPA funds through FY 2025-26. The PATH-STARS maximum compensation will increase from $4,161,520 to $4,169,968, which includes an additional $8,448 in HHAP dollars for FY 25-26, and $14,802 in unspent ARPA funds rolled over from FY 2024-25 to FY 2025-26. Sufficient appropriations and estimated revenues are included in the Department’s Org 5630 FY 2025-26 Adopted Budget and will be included in future budget requests for the duration of the Agreement term. 

 

DISCUSSION:


On June 22, 2021, the Board approved Agreement No. 21-214 with KV to deliver integrated mental and supportive housing services to adults who are homeless, or who are at imminent risk of becoming homeless, and have a serious mental illness and/or co-occurring disorder, to enable this population to live in the community and to avoid homelessness, hospitalization, and/or jail detention. On August 24, 2021, the Board approved Amendment No. 1 to Agreement No. 21-214 (No. 21-214-1) to utilize Community Development Block Grant-Coronavirus (CDBG-CV) and Coronavirus Aid, Relief, and Economic Security (CARES) Act funds to fund the Mobile Outreach Project through FY 2021-22. On May 17, 2022, the Board approved a superseding Agreement (No. 22-208) to replace Agreement No. 21-214.

 

On May 9, 2023, the Board retroactively approved and authorized the prior submittal of an Application to the Department of Health Care Services for the PATH grant effective July 1, 2023, through June 30, 2024 ($439,203), and delegated authority to the Department Director, or their designee, to be the signatory on all grant agreement documents. The Board also approved Amendment No. 1 to Superseding Agreement No. 22-208 (No. 23-206) to expand the dedicated mobile outreach team using other State homelessness funding (HHAP funding programs) to further extend the STARS component through June 30, 2026, using HHAP Rounds 1 - 4 allocated funds and ARPA funding.

 

On June 20, 2023, the Board approved Agreement (No. 23-295) to amend, replace, and supersede Agreement No. 22-208, as amended, with KV for California Advancing and Innovating Medi-Cal requirement updates including payment structure changes with extensions to the term. 

 

On July 9, 2024, the Board approved Amendment No. 1 to Agreement No. 23-295 with KV (No. 24-371) to revise contractual language, extend the term by one year, and increasing the maximum compensation using HHAP funding by $1,569,264 to a total of $4,161,520.

 

DSS has identified an additional $8,448 in available HHAP funding, which must be expended by December 31, 2025. In addition, the CAO has identified $14,802 in rollover ARPA funds from FY 24-25, which must be spent by May 31, 2026. Both funding sources will support expanded outreach, engagement, and service coordination for individuals experiencing homelessness in rural areas of Fresno County.

 

The STARS program delivers critical street-based outreach and support to individuals experiencing homelessness in remote and underserved regions. In FY 2024-25, the CAO partnered with KV through the County’s Homeless Encampment Resolution Workgroup, further enhancing the STARS program’s capacity to address homelessness in a coordinated, compassionate, and housing-focused manner. Between July 2024 and March 2025, STARS served 386 individuals, significantly exceeding the annual contractual requirement of 100.

 

The additional HHAP and ARPA funds are incorporated into the revised STARS budget, scope of work, and agreement language. These updates will support expanded services, strengthen interagency collaboration, and improve outreach effectiveness across the County.

 

The scope of work has also been updated to include the use of mobile showers during outreach and to modify the Specialty Mental Health Services (SMHS) component, allowing more individuals to be served. Additionally, portions of the Outreach Engagement Linkage (OEL) budget were modified, and the agreement’s modification language was replaced in its entirety to reflect updated Department and County agreement modification language. The modification clause delegates the Board’s authority to the DBH Director to make certain limited non-material changes to the agreement with no change to the maximum compensation.
