SUBJECT: Second Amendment to Agreement No. A-25-538 with BluePath Health, Inc.

RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute a Second Amendment to Agreement No. A-25-538 with BluePath Health, Inc. for additional consultation and implementation support services to the Probation Department on the California Advancing and Innovating Medi-Cal Justice-Involved Initiative, to extend the term from April 1, 2027 to December 31, 2027, and increase the maximum compensation by $299,754 to total not to exceed $919,719.
REPORT
There is no Net County Cost associated with the recommended action. Approval of the recommended action will allow BluePath Health, Inc. (BluePath) to continue to support the Probation Department (Department)'s implementation of California Advancing and Innovating Medi-Cal (CalAIM) Justice-Involved (JI) Reentry Initiative, which provides services to Medi-Cal-eligible individuals up to 90 days pre-release to ensure continuity of care. The cost of the recommended amendment will be fully funded through the CalAIM JI Providing Access and Transforming Health (PATH) Capacity Building Program grant. This item applies countywide.

ALTERNATIVE ACTION(S):

If the recommended action is not approved, the Department will be unable to continue to retain BluePath, which may result in noncompliance with CalAIM JI re-entry requirements and delay the Department's implementation efforts. The Department will also need to return PATH Round 4 grant funds.

FISCAL IMPACT:

There is no Net County Cost associated with the recommended action. The recommended amendment has an increase of $299,754 for additional services. The new total maximum compensation of $919,719 is fully funded by CalAIM PATH grant funds.
