RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute an Agreement with NMK Research and Consulting, for local tobacco control program evaluation services, effective December 11, 2018, not to exceed four years and seven months, which includes a two-year and seven-month base contract and two optional one-year extensions, total not to exceed $239,882.
REPORT
Approval of the recommended action will continue comprehensive evaluation services for the Department of Public Health's Tobacco Prevention Program (TPP) to satisfy the requirement under the California Department of Public Health's (CDPH) California Tobacco Control Program (CTCP), for a dedicated external evaluator. FY 2018-19 $36,509
FY 2019-20 $50,453
FY 2020-21 $52,920
FY 2021-22 $50,000
FY 2022-23 $50,000

The recommended agreement will be funded entirely by CTCP grant funds. Sufficient appropriations and estimated revenues are included in the Department's Org 5620 FY 2018-19 Adopted Budget and will be included in future budget requests for the duration of the term.
