SUBJECT: Retroactive Grant Application and Award with the California Department of Health Care Services

RECOMMENDED ACTION(S):
TITLE
1. Retroactively approve and authorize the Probation Department's previous submittal of a grant application to the California Department of Health Care Services for Providing Access and Transforming Health Justice-Involved Capacity Building Program Round 4 Grant related to the implementation of the Statewide California Advancing and Innovating Medi-Cal Initiative; and
2. Receive award and retroactively authorize the Chief Probation Officer or designee to electronically sign and submit the Acknowledgement of Grant Terms and Conditions form and execute any documents necessary with the Department of Health Care Services to accept and collect the grant in the amount of $300,000.
REPORT
Approval of the recommended actions will authorize the retroactive submission of a grant application to the California Department of Health Care Services (DHCS) through the California Advancing & Innovating Medi-Cal (CalAIM) Initiative for the Providing Access and Transforming Health (PATH) Justice-Involved (JI) Round 4. The funding provides Round 3 awardees with additional funding to support the planning and implementation of targeted pre-release Medi-Cal services to eligible individuals in correctional facilities, as outlined in the CalAIM Section 1115 Demonstration approval.

It should be noted that DHCS published a request for applications on May 12, 2025. In order to meet the application deadline of July 11, 2025, the Probation Department (Probation) submitted the application on July 9, 2025, contingent upon the Board's approval. DHCS issued the award notification on April 21, 2026, along with the Grant Terms and Conditions, which serve as the grant agreement. This item is countywide.
