SUBJECT: Revenue Agreement with the California Department of Public Health for the Type 2 Diabetes Prevention Program
RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute a revenue agreement with the California Department of Public Health for the Type 2 Diabetes Prevention Program to address chronic disease prevention, effective upon approval of California Department of General Services through June 30, 2028, total not to exceed $237,000; and
2. Approve and authorize the Director of the Department of Public Health, or designee, to execute the Contractor's Release for the final invoice and other forms as required by the proposed Agreement.
REPORT
There is no additional Net County Cost associated with the recommended actions. Approval of the first recommended action will allow the Department of Public Health (Department) to implement an agreement with California Department of Public Health (CDPH) to support the Department's effort to address chronic disease by providing prevention and health equity support services to populations of Fresno County with the highest health disparities. Approval of the second recommended action will allow the Director of the Department to execute the State Contractor's Release form required to be submitted with the final invoice to CDPH. This item is countywide.
ALTERNATIVE ACTION(S):
Should your Board not approve the recommended actions, the Department would be unable to provide type 2 diabetes intervention services to vulnerable community members.
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended actions. The recommended revenue agreement is funded by the CDPH Type 2 Diabetes Program (T2DP), provided by the Centers for Disease Control and Prevention (CDC).
