SUBJECT: Amendment to Agreement with Valley Children's Hospital

RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute First Amendment to Agreement No. 06-274 with Valley Children's Hospital to acknowledge the hospital's name change and revise new student screening requirements, effective upon execution, with no change to the unlimited one-year extensions ($0).
