SUBJECT: Retroactive Amendment to Agreement with the California Department of Public Health for Project LAUNCH

RECOMMENDED ACTION(S):
TITLE
Approve and authorize the Chairman to execute retroactive Amendment II to Agreement No. 16-502 with California Department of Public Health, to revise the scope of work and funding for the Project LAUNCH (Linking and Addressing the Unmet Needs for Children's Health) Program, effective July 1, 2017, extending the term by three months from June 30, 2019 to September 30, 2019 and decreasing the maximum compensation by $61,328 to a total of $306,640.
REPORT
Approval of the recommended action will extend the term of the agreement, replace the Scope of Work in its entirety to reflect the extended term, and clarify details to ensure compliance with the implementation and evaluation requirements of the California Department of Public Health's (CDPH) Project LAUNCH (Linking and Addressing the Unmet Needs for Children's Health) Program. The recommended amendment reduces funding for Budget Year Three ($30,664) and Budget Year Four ($57,032), and allocates additional funding for the three-month extension period ($26,368). This item is countywide.

RETROACTIVE AGREEMENT:

The recommended amendment was received from CDPH on April 10, 2019 and is retroactive to July 1, 2017 due to decreases in Budget Years Two and Three. The time required to prepare and review the recommended amendment did not allow presentation to your Board at an earlier date.

FISCAL IMPACT:

There is no increase in Net County Cost associated with the recommended action. The recommended amendment represents a $61,328 decrease from the original $367,968 amount. Amended funding by grant year is as follows:
