SUBJECT: Retroactive Revenue Agreement with the California Department of Public Health

RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute a retroactive revenue Agreement with the California Department of Public Health, for Prevention Forward interventions to address chronic disease prevention, effective April 1, 2019 through June 29, 2020 ($43,000).

2. Approve and authorize the Director of the Department of Public Health, or designee, to execute the State Contractor's Release for the final invoice.
REPORT
Approval of the recommended actions will support the Department of Public Health's effort to address chronic disease prevention throughout the County. The California Department of Public Health (CDPH) funding authorizes the Department to provide prevention and management support services to local communities with adult populations at high risk for or with a high prevalence of diabetes, with no increase in Net County Cost. This item is countywide.

ALTERNATIVE ACTION(S):

There are no viable alternative actions. Should your Board not approve the recommended actions, the Department would be unable to complete Prevention Forward intervention services.

RETROACTIVE AGREEMENT:

The recommended revenue agreement required revisions by CDPH, with the final received on May 6, 2019 and retroactive to April 1, 2019.

FISCAL IMPACT:

There is no increase in Net County Cost associated with the recommended actions. The recommended revenue agreement is funded by the CDPH for Prevention Forward interventions, provided by the Centers for Disease Control and Prevention (CDC). The allocation ($43,000) will pay for personnel costs, and program related services to prevent delay or control diabetes. Maximum compensation under the one-year and three-month recommend agreement shall not exceed:
