SUBJECT:                     Amendment to the Revenue Agreement with the California Department of Public Health for the Local Oral Health Program and Retroactive Second Amendment to Agreement with Fresno County Economic Opportunities Commission

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute Amendment to Revenue Agreement and Form 1229A to Agreement No. A-18-047 (State Agreement No. 17-10690, A02) with the California Department of Public Health, for the California Oral Health Program Local Oral Health Plan, effective upon execution with no change to the term of February 6, 2018 through June 30, 2022 and increasing the maximum by $98,647 to a total of $2,742,927.

 

2.                     Approve and Authorize the Chairman to execute a retroactive Second Amendment to Agreement No. A-20-025 with the Fresno County Economic Opportunities Commission to continue the implementation of a school-based sealant program and modify the budget, effective upon execution with no change in the term of January 7, 2020 through June 30, 2022, or compensation maximum of $520,000.

REPORT

There is no additional Net County Cost associated with the recommended actions. Approval of the first recommended action will increase the grant maximum by $98,647 to enhance current activities being performed that support in school-linked dental programs equipment purchases and referral management software purchases.  Approval of the second recommended action will move unspent funds to FY2021-22 operational expenses to allow Fresno County Economic Opportunities Commission (Fresno EOC) to continue providing dental sealants to school aged children in the County while continuing to work on completion of grant activities.  This item is countywide.

 

ALTERNATIVE ACTION(S):

 

Should your Board not approve the recommended actions, the Local Oral Health Program (LOHP) would not be funded for dental equipment purchases and referral management software purchases to support the Children’s Dental Disease Prevention Program and the Fresno EOC agreement would remain unchanged which would limit the number of school aged children receiving dental sealants.   

 

RETROACTIVE AGREEMENT:

 

The proposed second amendment is retroactive to June 30, 2021 to modify the previous FY 2020-21 budget to actual expenditures and shift unspent funds to the current FY 2021-22 budget without increasing the maximum compensation.

 

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended actions. Approval of the first recommended action increases the funding of the LOHP grant from $2,644,280 to $2,742,927, an increase of $98,647, to help support our school-linked dental programs with equipment and referral management software purchases, during the current fiscal year. 

 

Approval of the second recommended action will revise Fresno EOC year two and three budgets with no change to the maximum compensation ($520,000) as follows:

 

Year I (Jan. 7, 2020-June 30, 2020) $81,643 (no change)

Year II (July 1, 2020-June 30, 2021) $219,765 (reduced $23,592)

Year III (July 1, 2021-June 30, 2022) $218,592 (increased $23,592)

 

Sufficient appropriations and estimated revenues are included in the Department’s Org 5620 FY 2021-22 Adopted Budget.

 

DISCUSSION:

 

On February 6, 2018, the Board approved a five-year agreement with the California Department of Public Health (CDPH) for a LOHP grant with five annual allocations of $528,856 each; total compensation of $2,644,280. 

 

On September 24, 2019 the Board approved a first amendment to the oral health grant to revise the five annual allocations into one lump sum amount and for the revision of the grantee name from “Fresno County Department of Public Health” to “County of Fresno”, without increasing the maximum compensation or terms of the agreement. 

 

The recommended amendment A02 will allow the LOHP to enhance the children’s dental disease prevention program by funding:

                     a school-linked software program for referral management and care coordination, to connect with parents and link children to a source of dental care that tracks the progress of care from referral to completion of treatment.

                     a school-based program to provide the necessary dental equipment for the delivery of sealants and other oral health treatments to reduce the burden of oral diseases.  Additional equipment will expand the school dental programs therefore increasing access for children.

 

Approval of the second recommended action will allow Fresno EOC to move unspent funds to operational expense line items in the current fiscal year budget to ensure the funding to complete unfinished activities is accessible while they continue to provide elementary school aged children in grades K-6 fluoride varnishes or dental sealants.  All other agreement terms remain unchanged.

 

Risk Management reviewed and noted that the insurance requirements and recommended indemnification language are different from the County’s standard insurance requirements but believes the risk to be acceptable because we are recipients of grant funds being issued by the State of California.
