SUBJECT: Revenue Agreement with California Department of Public Health for the Nutrition Education and Obesity Prevention Grant and Amendments for Direct Nutrition Education and/or Community Engagement Services
RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute revenue Agreement with the California Department of Public Health for a Nutrition Education and Obesity Prevention Grant to provide nutrition education and community engagement services, effective October 1, 2016 through September 30, 2019 ($5,999,043).
2. Approve and authorize the Chairman to execute Amendment II to Agreement No. 13-721 with Public Health Institute, an increase of $80,578, to provide nutrition education and community engagement services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($1,023,623).
3. Approve and authorize the Chairman to execute Amendment II to Agreement No. 13-722 with Fresno County Superintendent of Schools through its Office of Education, an increase of $70,190, to provide nutrition education services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($951,417).
4. Approve and authorize the Chairman to execute Amendment I to Agreement No. 13-723 with California Health Collaborative, an increase of $58,749, to provide nutrition education and community engagement services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($817,068).
5. Approve and authorize the Chairman to execute Amendment I to Agreement No. 13-724 with Community Food Bank, an increase of $25,198, to provide nutrition education and community engagement services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($448,397).
REPORT
Approval of the first recommended action will allow the Department of Public Health to continue to fund Nutrition Education and Obesity Prevention (NEOP) activities throughout the County by supporting County NEOP staff, supplies and service contracts, with no increase in Net County Cost. The revenue Agreement was received from the California Department of Public Health (CDPH) on August 10, 2016.
Approval of the second through fifth recommended actions will allow continuity of direct nutrition education and community engagement services by extending existing contracts through December 31, 2016 and utilize $234,715 of the NEOP federal fiscal year (FFY) 2016-17 revenue allocation beginning October 1, 2016, with no increase in Net County Cost.
New service contracts from a County Request for Proposals (RFP) are expected to be presented for your Board’s consideration on December 13, 2016 with an anticipated effective date of January 1, 2017.
ALTERNATIVE ACTION(S):
Should your Board not approve the first recommended action, the Department would be unable to fund Nutrition Education and Obesity Prevention services throughout the County.
Should your Board not approve the second through fifth recommended actions, service contracts will end on September 30, 2016, resulting in a three-month lapse in services to the community until new service provider contracts become effective on January 1, 2017, pending your Board’s approval.
FISCAL IMPACT:
There is no increase in Net County Cost. The first recommended action authorizes the Department to execute a three-year revenue Agreement with CDPH effective October 1, 2016 through September 30, 2019. The annual maximum compensation is $1,999,681; $5,999,043 for the term. Sufficient appropriations and estimated revenues are included in the Department Org 5620 FY 2016-17 Adopted Budget and will be included in subsequent fiscal year budget requests. The Department’s indirect rate is 15.873 percent; however, all agreements with CDPH have a maximum indirect rate of 15.0 percent, or an estimated $299,952 for the first year; the remainder will be covered using Health Realignment funds.
Approval of the second through fifth recommended actions will allow for a continuity of direct services to the targeted communities extending from October 1, 2016 through December 31, 2016. Sufficient appropriations and estimated revenues are included in the Department Org 5620 FY 2016-17 Adopted Budget to cover these services. The recommended Amendments utilize $234,715 of the NEOP federal fiscal year (FFY) 2016-17 revenue allocation beginning October 1, 2016. Years two and three remain unchanged.
The maximum compensation for the extended Agreements is as follows:
• Public Health Institute (PHI) increased by $80,578 to $1,023,623, a maximum third year compensation of $451,452;
• Fresno County Superintendent of Schools, through its Office of Education, (FCOE) increased by $70,190 to $951,417, a maximum third year compensation of $300,152;
• California Health Collaborative (CHC) increased by $58,749 to $817,068, a maximum third year compensation of $311,522;
• Community Food Bank (CFB) increased by $25,198 to $448,397, a maximum third year compensation of $166,264.
DISCUSSION:
The County has been receiving NEOP grant funds since 2013. The overall goal of the NEOP Program is to provide nutrition education to Supplemental Nutrition Assistance Program Nutrition-Education (SNAP-Ed) eligible County residents (up to 185 percent Federal Poverty Level) to increase consumption of health foods and beverages, reduce consumption of less healthy foods and beverages and increase physical activity in six targeted County communities: Kerman, Lowell, Parlier, Reedley, Sanger, and West Fresno.
Approval of the first recommended action will allow the Department to continue to build community awareness to improve the health of County residents by providing nutrition education. The NEOP Program interventions include:
• increase access to fruits, vegetable and other healthy food option in the six targeted communities;
• develop public awareness campaigns promoting healthy food and beverage options;
• promote optimal health and reduce chronic disease risks; and,
• increase resources that promote physical activity and link low-income families with nutrition resources, such as CalFresh and Women, Infants, and Children (WIC) programs.
Contracts from the pending RFP will fulfill NEOP scope of work requirements and are anticipated to be effective January 1, 2017 through September 30, 2019. The Department will return to your Board for approval of new Agreements on December 13, 2016. The Department has received approval from CDPH to amend and extend current contracts utilizing funds from year one of the CDPH revenue Agreement in order to provide service continuity to the community until the Department identifies new contractors.
On December 3, 2013, your Board approved the following three-year Agreements funded with NEOP dollars: (1) PHI, (2) FCOE, (3) CHC, and (4) CFB effective November 12, 2013 through September 30, 2016 to fulfill NEOP community engagement scope of work requirements. Approval of the second through fifth recommended actions will allow the Department to continue the following nutrition education interventions in the targeted communities:
• PHI will continue to recruit and train community leaders to implement nutrition and physical activity community engagement projects with adults and youth. PHI will continue to conduct community outreach with youth and adults to identify and determine what healthy changes are needed to increase access and consumption of health foods and beverages and promote physical activity opportunities.
• FCOE will continue to provide nutrition education and physical activity interventions in schools, after-school programs and youth engagement projects including classroom and parent education.
• CHC will continue to recruit and work with retailers to promote and implement nutrition education interventions in retail settings. CHC will also continue to implement worksite wellness activities with participating worksites.
• CFB will continue to provide direct and indirect nutrition education by using the CFB Nutrition on Wheels (NOW) during food distributions and at community based organizations.
The contractors’ scopes of work will be amended to include additional reach numbers and maintain current nutrition education services for the extension period as appropriate. No new activities will be included in the amended NEOP scope of work.
