RECOMMENDED ACTION(S):

Approve and authorize the Chairman to execute Amendment III to Purchasing Agreement P-15-007-O with John E. Goodfellow, dba Goodfellow Occupational Therapy Services, to add pediatric on-site physical therapy services, effective July 1, 2017 to June 30, 2019 and increasing the maximum by $126,350 to a total of $3,477,810.

REPORT

Approval of the recommended action will amend the existing agreement with John E. Goodfellow (Goodfellow) and expand the responsibilities to include on-site pediatric physical therapy services, a California Department of Health Care Services (DHCS) mandated component of the California Children’s Services (CCS) Medical Therapy Program (MTP).  County Human Resources and the Department of Public Health have had an ongoing recruitment for Physical Therapists since July of 2015 but have had difficulty filling the two (2) vacant Physical Therapist positions.  The proposed amendment will be funded with DHCS and Health Realignment monies, with no Net County Cost.

 

ALTERNATIVE ACTION(S):

 

Should your Board not approve the recommended action, the Department will continue to maintain a physical therapy waitlist, which currently includes 15 children and requires approximately 665 work hours (annually) to address. Service delays will increase as more children needing physical therapy enter the program if recruitment efforts continue to be unsuccessful.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action. The recommended amendment will increase the maximum compensation from $3,351,460 to $3,477,810 ($126,350) ($63,175 in both FY 2017-18 and FY 2018-19) for a total of $3,477,810. Funding for the MTP is provided by DHCS (50%) and Health Realignment (50%). Sufficient appropriations and estimated revenues will be included in the Department of Public Health Org 5620 FY 2017-18 Requested Budget and will be included in subsequent budget requests. Actual charges will be based on actual services performed.

 

DISCUSSION:

 

The MTP is a specialty program within California Children’s Services that provides medically necessary outpatient physical therapy (PT), occupational therapy (OT), and medical therapy conference (MTC) services for children with disabilities and special needs conditions, generally due to neurological or musculoskeletal disorders. These services are provided at Medical Therapy Units (MTUs).

 

Since November 18, 2014, Goodfellow has been contracted to provide on-site pediatric occupational therapy services by qualified therapists at the County’s three MTU locations.  Due to the continued Occupational Therapist recruitment challenges and the need for pediatric Occupational Therapists, the Goodfellow sole source agreement term and compensation was extended and increased by your Board (June 2, 2015 Agreement No. 15-220 and May 10, 2016 Agreement No. 16-189). Goodfellow is the only vendor in the area with the expertise and adequate staff required to perform the needed services.

 

The recommended amendment is a result of the ongoing difficulty in recruiting Physical Therapists and a continued need for pediatric services. The Department has had a continuous recruitment for Physical Therapists since July of 2015. There has only been one applicant since its opening, which was received in March of 2016; however, the candidate declined the job offer. While the MTP does have four full-time County employed Physical Therapists, they are unable to meet present demand and there are currently 15 children that have been put on a waiting list. These children will not be able to receive their State-mandated services until such time that the MTP can recruit at least one Physical Therapist. This waiting list equates to the need of approximately 665 hours (annually) in order to address. Should the MTP lose any of their current Physical Therapists, the waiting list would increase.

 

With your Board’s approval, the MTP will be able to utilize the Physical Therapists provided by Goodfellow at the compensation rate $95 per hour to address the current waitlist and future demand until such time that the Department can recruit its own therapists.
