SUBJECT:                     Amendment to Agreement with the California Department of Public Health for the California Home Visiting Program

 

RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute Amendment I to Agreement No. 15-266 with the California Department of Public Health for the California Home Visiting Program, effective April 25, 2017 with no change to the term through June 30, 2019 and decreasing the maximum by $11,428 to a total of $3,573,928.

REPORT

Approval of the recommended action will decrease funding in the California Home Visiting Program (CHVP), by 3% ($11,428) due to the California Department of Public Health’s (CDPH) amendment to the scope of work, which revised and clarified the main goals.  The decrease will not impact the services CHVP provides including public health nurse home visitation, health education and linkage to community resources to first-time, low-income mothers and their families in Selma and the surrounding rural areas. 

 

ALTERNATIVE ACTION(S):

 

No viable options are available.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action.  The recommended amendment will decrease the revenue agreement from $3,585,356 to $3,573,928, a 3% decrease ($11,428).  The reduction impacts Year 2 (FY 2016-17), reducing the maximum from $863,839 to $852,411, and adjusts line items to coincide with the reduction.  Sufficient appropriations and estimated revenues are included in the Department’s Org 5620 FY 2016-17 Adopted Budget. 

 

DISCUSSION:

 

On June 16, 2015, your Board approved Agreement No. 15-266 with CDPH to continue the CHVP Nurse-Family Partnership (NFP) home visiting services in Selma and surrounding rural areas of the County.  The agreement allows the County to assist low-income, first-time mothers and their families to achieve positive outcomes such as:

 

                     Reduction of emergency department visits, crime and domestic violence;

                     Improvement in school readiness and achievement;

                     Family economic self-sufficiency; and,

                     Coordination of referrals for other community resources and supports.

 

The recommended amendment revises the original scope of work clarifying the goals as follows:

 

1.                     Provide leadership and structure for implementation of the CHVP at the Local Health Jurisdiction (LHJ) site.

2.                     Embed the LHJ site into a well-integrated local early childhood system of services.

3.                     Monitor federal benchmark measures to show improvement in maternal and early childhood health.

 

The revision will ensure compliance with implementation and evaluation requirements of the Maternal, Infant and Early Childhood Home Visiting grant, which funds CHVP.  The revision and clarification of the goals will not impact the services provided.  The recommended amendment also reflects a Year 2 funding decrease of $11,428 and line item shifts on Attachment A (Exhibit B) to accommodate actual implementation of CHVP. 

 

CDPH determined base allocation reductions levels using the following criteria:

 

                     Unspent FY 2014-15 funds;

                     Caseloads below capacity over a 12-month period; and

                     Chronic staffing vacancies.

 

Approval of the recommended action will enable the County to continue providing CHVP NFP services to families in the rural areas, guiding them in nurturing their children, future family planning, continuing their education and obtaining employment.
