SUBJECT: Retroactive Agreements for Local Dental Pilot Project
RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute a retroactive Master Agreement for dental provider participation in the Local Dental Pilot Project and associated training stipends, effective July 1, 2017 through December 31, 2020 ($49,896).
2. Authorize the Director of the Department of Public Health, or designee, to add and/or delete qualified dental providers to the Master Agreement, as necessary.
3. Approve and authorize the Chairman to execute a retroactive Agreement with Fresno Economic Opportunities Commission - Health Services Education for administrative and staffing services required to conduct the Local Dental Pilot Project, effective July 1, 2017 through December 31, 2020 ($5,124,010).
4. Approve and authorize the Chairman to execute a retroactive Agreement with Reading and Beyond - Health Services Education for administrative and staffing services required to conduct the Local Dental Pilot Project, effective July 1, 2017 through December 31, 2020 ($4,936,865).
5. Approve and authorize the Chairman to execute a retroactive Agreement with the University of the Pacific School of Dentistry for dental provider training and consultation services required to conduct the Local Dental Pilot Project, effective July 1, 2017 through December 31, 2020 ($332,626).
REPORT
There is no increase in Net County Cost associated with the recommended actions. The first recommended action will allow the Department of Public Health to contract with community-based dentists that will operate the Local Dental Pilot Project (Pilot Project) Virtual Dental Homes (VDH) through participation stipends. The second recommended action will allow the Department Director, or designee, to add and/or delete dental providers from the recommended agreement, as appropriate. Recommended actions three through five will allow the Department to contract with participating entities for administrative and staffing services and dental provider training and consultation services.
ALTERNATIVE ACTION(S):
Your Board may direct the Department to issue a request for proposals to develop individual agreements with dental providers in lieu of the recommended master agreement; however, doing so would delay needed dental provider services to County residents by a minimum of six months. If recommended actions three through five are not approved, the Pilot Project may be jeopardized as the vendors were integral in the proposal development and were approved by the Department of Health Care Services (DHCS) as key partners.
RETROACTIVE AGREEMENTS:
The recommended agreements are retroactive to July 1, 2017 and were not presented to your Board with revenue Agreement No. 17-298 on June 20, 2017 due to delays in contract negotiations and vendor review.
FISCAL IMPACT:
There is no increase in Net County Cost associated with the recommended actions. The maximum compensation for each recommended agreement will be as follows.

The recommended agreements are fully reimbursable through the DHCS Dental Transformation Initiative (DTI) Pilot Project grant, approved by your Board on June 20, 2017 in the amount of $11,127,285, with no required matching funds. The Department will retain $683,888 for administrative costs. Sufficient appropriations and estimated revenues will be included in the Department’s Org 5620 FY 2017-18 Recommended Budget and in subsequent budget requests for the term of the agreements.
DISCUSSION:
On June 20, 2017, your Board approved revenue Agreement No. 17-298 for the Pilot Project grant. With your Board’s approval, the Pilot Project will provide preventive dental services to Medi-Cal enrolled children aged 0-20 in areas of the community where people live, work, play, go to school, and receive social services through the implementation of mobile VDHs.
The EOC, RAB, UOP, and two local dental providers collaborated in the formation of the Pilot Project proposal as key partners. During DHCS’s DTI application opportunity posted on their website from December 2015 through September 2016, no other entities, outside of the key partners, contacted the Department to express interest in participating in the project proposal. Furthermore, the Department is the only entity in the County area that applied for DTI Pilot Project funds.
The recommended agreements with EOC and RAB will provide administrative and staffing services for the oral health education and outreach initiatives as well as location procurement for the VDHs. The recommended agreement with UOP will allow for dental provider training and consultation services regarding the VDH operation. The recommended master agreement will allow stipends to be issued to dental providers for participating in UOP trainings throughout the term. The Department will be responsible for the purchase of the VDH equipment, conducting a media campaign, and monitoring the performance metrics to ensure the success of the Pilot Project, including hosting a monthly advisory board meeting designed to address any issues that arise while conducting the project.
