RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute a revenue agreement and certifications with the California Department of Public Health for Local Oral Health Plan funds, effective upon execution through June 30, 2022 ($2,644,280).

REPORT

Approval of the recommended agreement with the California Department of Public Health (CDPH) will allow the Department of Public Health to accept grant funds to complete a local health assessment, improvement plan and action plan with relevant oral health activities, with no increase in Net County Cost. This item is Countywide.

 

ALTERNATIVE ACTION(S):

 

There is no viable alternative action. Should your Board not approve the recommended action, the Department would be unable to fund oral health activities as outlined in the recommended agreement.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action. The recommended revenue agreement will provide $528,856 per fiscal year for five years (FYs 2017-18 through 2021-22), for a maximum of $2,644,280. CDPH limits indirect cost recovery to 25% of personnel costs as compared with the Department’s full indirect rate of 26.19%. The remainder ($764 in year one) will be covered using Health Realignment funds. Sufficient appropriations and estimated revenues were included in the Department’s Org 5620 FY 2017-18 Adopted Budget, and will be included in future budget requests for the duration of the term.

 

DISCUSSION:

 

On November 14, 2017, your Board approved the Department’s non-competitive grant application to CDPH for the new Local Oral Health Plan grant. On December 5, 2017, the Department received the recommended agreement from CDPH to develop, implement, and evaluate best practice and evidence-based health and oral health planning, programs and services. With your Board’s approval, the Department will use the grant to fund staff, services, and supplies to complete the following activities over the five-year term:

 

                     conduct a needs assessment to determine County residents’ health status, oral health status, and available medical and dental services and resources with an emphasis on underserved and vulnerable populations;

                     create a five-year health improvement plan and action plan addressing oral disease prevention, surveillance, education, linkage to treatment programs, and evaluation strategies;

                     coordinate educational outreach programs and health literacy campaigns, and implement best practice policies and guidelines to integrate oral health and primary care; and,

                     engage community stakeholders to achieve large-scale improvements in the health system, evidence-based interventions, community-clinical linkages, and disease surveillance and evaluation.

 

Funding for this project comes from California’s Proposition 56: Cigarette Tax to Fund Healthcare (2016), and is expected to continue beyond the initial five-year term. The Department is working with CDPH to determine the best short-, medium- and long-term strategies to effectively implement this new grant program. Specific activities and actions will be identified during the assessment and planning phase, scheduled for completion by December 2018. The Fresno Community Health Improvement Partnership, with Fresno Metro Ministry as its sponsor, will lead the collaborative assessment and planning process. To complete required activities, the Department will return to your Board in FY 2018-19 with related agreements for a school-based oral health services contractor and an evaluation contractor.

 

The recommended agreement requires that the County indemnify CDPH regarding claims and losses resulting to third parties providing services, materials, or supplies related to the grant project without reciprocal CDPH indemnification of the County.  The recommended agreement also contains non-standard termination language as it allows CDPH to terminate without cause, if written notice has been delivered at least 30 days prior to termination. The County may submit a written request to terminate the agreement only in the event CDPH substantially fails to perform its responsibilities under the agreement. In the event of a dispute between parties, the resolution process outlined in the agreement terms and conditions must be followed.
