SUBJECT: Fresno County Preterm Birth Initiative Forms
RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize Department of Public Health employees identified as having a need to participate in the Fresno County Preterm Birth Initiative committees and workgroups to execute a Confidentiality Statement Form.
2. Approve and authorize Department of Public Health employees identified as having a need to participate in the Fresno County Preterm Birth Initiative committees and workgroups to execute a Visual/Audio Image Release Form with Central California Center for Health and Human Services at California State University, Fresno.
REPORT
Approval of the recommended actions will allow Department of Public Health identified employees participating in the Fresno County Preterm Birth Initiative (PTBi) committees and workgroups to complete required participation forms. The Fresno County PTBi is a cross-sector, collaborative effort already providing specific support and services to communities that are at highest risk for premature birth, with no increase in Net County Cost. This item is countywide.
ALTERNATIVE ACTION(S):
There is no viable alternative action. Should your Board not approve the recommended actions, the Department would not be able to participate in the Fresno County PTBi committees and workgroups that require completed forms.
FISCAL IMPACT:
There is no Net County Cost associated with the recommended action. The support and services to communities are already being provided through other sources of funding and employees will complete the forms as part of their regular assigned duties.
DISCUSSION:
Preterm birth is the leading cause of infant poor health, disabilities, and death. The Fresno County PTBi is a cross-sector, collaborative effort providing specific support and services to communities that are at highest risk for premature birth. Through PTBi, institutions and organizations across the County are working together to improve the coordination of programs and services, as well as introduce new and innovative practices to reduce preterm births in Fresno County from 11.1% to 7% by 2025.
Department employees participate in Fresno County PTBi committees and workgroups as part of the collaborative effort. The recommended actions will allow identified Department employees to complete a Confidentiality Statement Form and Visual/Audio Image Release Form. The Visual/Audio Image Release Form releases Fresno State from any claims, damages or liability in connection with the use/publication of the visual/audio images. Both forms are required by Fresno County PTBi for each committee and workgroup.
