SUBJECT:                     Amendment III to Turning Point Agreement Agt. 13-273 and Agreement with Central Star Behavioral Health, Inc. for Transition Age Youth Mental Health Services, Supports and Housing Services

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute an Amendment III to Agreement No. 13-273 with Turning Point of Central California, Inc. extending the term by two months from November 1, 2018 through December 31, 2018, and increasing the maximum by $288,341 to a total of $14,170,378.

 

2.                     Approve and authorize the Chairman to execute an Agreement with Central Star Behavioral Health, Inc. for the provision of mental health services and supports and housing to the Transition Age Youth population, effective upon execution through June 30, 2024, which includes a three-year and eight-month base contract and two optional one-year extensions, total not to exceed $15,988,439.

REPORT

Approval of the first recommended action will allow for a two-month term extension with the current provider of integrated mental health and supportive housing services to Transition Age Youth (TAY), ages 16 to 25, Turning Point of Central California, Inc. (Turning Point), the complete transition of its remaining caseload of youth and young adults with serious emotional disturbance (SED) or serious mental illness (SMI) to the new provider of TAY services, and the close out operations for the Turning Point TAY program.

 

Approval of the second recommended action will allow the TAY Full Service Partnership (FSP) level services to be provided, as awarded via a competitive bid process, Request for Proposal (RFP) No. 18-038, by Central Star Behavioral Health, Inc. (Central Star), with no increase in Net County Cost.

 

ALTERNATIVE ACTION(S):

 

There is no viable alternative action. If your Board does not approve the recommended actions, the County will not be able to receive continuing services.  Without approval of these recommended actions, the TAY population in need of these services may experience a gap and/or delay in receiving essential mental health services, supports, and housing.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended actions. The increase for the Amendment III to the Turning Point Agt. No. 13-273 will be $288,341 and the maximum amount of the recommended agreement with Central Star ($15,988,439), for the entire contract period, will be fully offset with Mental Health Services Act (MHSA) Community Services and Supports (CSS) funds as well as Medi-Cal Federal Financial Participation (FFP), Mental Health realignment, and client rent. Sufficient appropriations and estimated revenues are included in the Behavioral Health Org 5630 FY 2018-19 Adopted Budget and will be requested in subsequent fiscal year budgets for the duration of the contract term.

 

DISCUSSION:

 

On May 21, 2013, the Board approved Agreement No. 13-273 with Turning Point of Central California, Inc. for the provision of the Transition Age Youth (TAY) Full Service Partnership (FSP) Program for the provision of integrated mental health services, supports, and housing. TAY FSP services were established for the TAY population with SED or SMI who were homeless or at risk of homelessness, frequent users of crisis services, and/or incarcerated (including those aging out of the Juvenile Justice System). The TAY FSP program was a “whatever it takes” program comprised of a welcoming, recovery-oriented, integrated, service delivery model that uses innovative interventions to reduce crisis services, admissions into inpatient facilities, or the criminal justice system. 

 

On March 20, 2018, the Department issued RFP No. 18-038 for Transition Age Youth Services to approximately 55 prospective bidders and was released on Purchasing’s Public Purchase site. An addendum was released on April 10, 2018, and the RFP closed on April 24, 2018 with four bids received. An evaluation panel convened on June 18, 2018 and consisted of the following members: two DBH Clinicians, a DBH Clinical Supervisor, a DBH Fiscal Analyst, and a Department of Social Services Social Work Supervisor. The review panel reached a consensus to recommend the contract award to Central Star based on a ranking system comparative to the other proposals. The evaluation panel unanimously awarded Central Star the contract due to bid responsiveness, depth of knowledge, 20 years of experience with the TAY population, 11 years of experience operating TAY FSP programs in six California counties, as well as their level of commitment to TAY services and the “whatever it takes” philosophy.

 

The current Agreement expires on October 31, 2018. In order to alleviate gaps in the continuum of care for the TAY population, the Department is recommending your Board approve a two-month extension period for the existing provider, Turning Point, while Central Star is able to ramp up their services, become Medi-Cal site certified, and begin accepting client transitions and referrals. Both agencies are dedicated to the ongoing services to the high acuity TAY population.

 

The recommended agreement with Central Star is effective upon execution through June 30, 2022. This initial period includes a three year and eight month period, which was extended from the usual three year period due to the recent transition of the already vulnerable TAY population and to align it with the fiscal year. The agreement may be extended for two additional consecutive 12-month periods upon written approval of both parties no later than 60 days prior to the first day of the next 12-month extension period. The DBH Director, or designee, is authorized to execute such written approval on behalf of the Contractor’s satisfactory performance.
