RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute an Agreement with Comprehensive Youth Services of Fresno, Inc. for the provision of Function Family Therapy Mental Health Prevention and Early Intervention Services for children/youth ages 11-18 years effective January 1, 2019 through June 30, 2023, which includes a two-year and six-month base contract and two optional one-year extensions, total not to exceed $9,697,943.

REPORT

Approval of the recommended action will allow for the provision of Mental Health Services Act funded mental health prevention and early intervention functional family therapy services to children/youth ages 11-18 years with no increase in Net County Cost.

 

ALTERNATIVE ACTION(S):

 

There are no alternative actions. Should the recommended action not be approved, there will be an interruption in mental health services for approximately 450 children and youth. These services have followed the competitive bid process and funding is available through Mental Health Services Act (MHSA) funds.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action. The maximum cost of the recommended agreement ($9,697,943) will be funded with Mental Health Services Act (MHSA) Prevention and Early Intervention (PEI) funds, Early Periodic Screening Diagnostic Treatment (EPSDT) revenue and Medi-Cal Federal Financial Participation (FFP).  Sufficient appropriations and estimated revenues ($1,051,983) are included in the Department’s Org 5630 FY 2018-19 Adopted Budget and will be included in subsequent budget requests for the duration of the contract term.

 

DISCUSSION:

 

Under the provisions of the Mental Health Services Act (MHSA), also known as Proposition 63, County mental health departments have received funding to assist in providing services to underserved and unserved populations through new or expansion of existing programs. The Department conducted a mandated local community stakeholder process to identify highest service needs of the County and accordingly allocate MHSA prevention and early intervention (PEI) funds to meet those needs. 

 

Since April 20, 2007, the County has had an agreement with Comprehensive Youth Services, Inc. (CYS) for mental PEI services to identified clients (specifically those that are at-risk of involvement or are currently involved in the juvenile justice system and have a significant family conflict) that are youth ages 11-18 utilizing the Functional Family Therapy (FFT) evidence-based model. 

 

A RFP No. 18-036 was issued on April 10, 2018 and closed on April 30, 2018. The Department received one bid submitted by Comprehensive Youth Services of Fresno, Inc. The review committee consisted of three Staff Analysts and one Clinical Supervisor from the Department of Behavioral Health, one Staff Analyst from the Department of Social Services.

 

CYS’s proposal demonstrated knowledge of the services requested. CYS possesses extensive experience having provided the services since 2007. CYS has a background working with the target population and delivering these services to rural communities maintaining flexible hours to help meet clients in the most appropriate setting and giving them the best opportunity for success.

 

CYS demonstrated an adherence to the fidelity of the FFT model and exhibited a strong record of delivering high quality services as shown by their extensive participation in FFT research and evaluation projects conducted on a national level. CYS employs staff that are diverse, well-versed in the delivery of FFT services, and are supported by close supervision.

 

CYS’s proposal included extensive experience with Medi-Cal billing and indicates an adequate support staff to manage Medi-Cal billing requirements. The proposal also demonstrates a deep understanding of MHSA PEI programs and all of the accompanying reporting requirements.

 

Under the Agreement, the contractor will maintain a caseload of 200 youth at any given time. Approximately 400 unique clients will be served each fiscal year. Services will be provided to clients who are at-risk of involvement, or are currently involved in the juvenile justice system and have a significant family conflict. Additionally, identified clients’ siblings, other relatives, parents, caregivers, and other significant support persons may participate and receive specialty mental health services from this program.

 

The recommended Agreement allows the Director to terminate without cause with 60-day advance written notice, authorize budget line item changes that do not exceed ten percent (10%) of the total annual contract amount.  Upon approval by your Board, the recommended action will be effective January 1, 2019.

The Behavioral Health Board was informed of the recommended agreement at its November 21, 2018 Board Meeting.
