SUBJECT:                     Agreement with California Department of Public Health Office of AIDS

RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute a revenue Agreement with California Department of Public Health Office of AIDS for the HIV Prevention State Grant to supplement and enhance HIV prevention activities, effective upon execution through June 30, 2019, total not to exceed $121,252.

REPORT

Approval of the recommended action will provide the California Department of Public Health Office of AIDS (CDPH-OA) HIV Prevention State Grant augmentation allocation to the Department of Public Health to supplement and enhance existing HIV (human immunodeficiency virus) prevention program activities and services in the local health jurisdiction, with no Net County Cost.  The recommended agreement is for one-time funding that will offset the related salary and benefits, training, and media campaign costs.  This item is countywide.

ALTERNATIVE ACTION(S):

Should your Board not approve the recommended action, the Department would not receive the augmentation allocation to enhance its HIV prevention program activities and services in FY 2018-19.

FISCAL IMPACT:

There is no increase in Net County Cost associated with the recommended action.  The allocation ($121,252) will offset salary and benefit costs, provide staff training, and fund a media campaign to enhance HIV prevention program activities.  Sufficient appropriations and estimated revenues are included in the Department’s Org 5620 FY 2018-19 Adopted Budget.  The award includes full reimbursement for direct costs and up to 25% or $18,500 of personnel costs as compared with the Department’s full indirect rate of 26.5%.  The difference 1.5% or $1,110 will be covered with Health Realignment.

DISCUSSION:

The Department offers HIV Prevention services at low or no cost to the individuals.  Services include, but are not limited to, HIV Prevention Education, rapid testing, linkage to care and counseling services.  In 2019, the Department’s HIV Prevention Program plans to pilot HIV outreach, education, and prevention efforts at the existing needle exchange clinic run by a community provider to reach another high-risk group in the community.  Pre-Exposure Prophylaxis (PrEP) Navigation services and provider detailing with PrEP is an ongoing focus of the program to increase resistance of HIV in at-risk individuals.

On August 14, 2018, CDPH-OA issued a letter stating the County was awarded funds for FY 2018-19 for evidence-based public health activities to address HIV prevention within the local health jurisdiction.  The proposed scope of work includes activities that allow the Department to provide greater prevention opportunities, reach higher risk individuals, and offer testing and prophylaxis opportunities to reduce the risk of contracting HIV.

The Department’s proposed activities include the following from the CDPH-OA HIV Strengthening Our Guidance Through Integration: 2019 Guide to HIV Prevention and Surveillance:

                     Strategy A: Improve PrEP Utilization - the assistance program allows high-risk clients access to pre-exposure medications that help the client become resistant to the HIV infection

                     Strategy B: Increase and Improve HIV Testing (routine, opt-out

                     Strategy C: Expand Partner Services

                     Strategy D: Improve Linkage to Care

                     Strategy K: Increase and Improve HIV Prevention and Support Services for People who Use Drugs

The Department’s proposed activities also include the following from the CDPH-OA Laying the Foundation for Getting to Zero: California’s Integrated HIV Surveillance, Prevention, and Care Plan:

                     Strategy H: Improve Integration of HIV Services with STD (sexually transmitted disease), TB (tuberculosis), Dental, and Other Health Services

The recommended agreement contains non-standard termination language as it allows CDPH-OA to terminate for cause, cancel immediately for cause, or upon 30 days written notice, the CDPH-OA may terminate with or without cause.  CDPH-OA may, at its discretion, require the County to cease portions of its scope of work while completing other components prior to the termination date of the proposed agreement.
