SUBJECT:                     Retroactive Revenue Agreement with the California Department of Public Health

 

RECOMMENDED ACTION(S):

TITLE

1.                     Approve and authorize the Chairman to execute a retroactive revenue Agreement with the California Department of Public Health, for Prevention Forward interventions to address chronic disease prevention, effective April 1, 2019 through June 29, 2020 ($43,000).

 

2.                     Approve and authorize the Director of the Department of Public Health, or designee, to execute the State Contractor’s Release for the final invoice.

REPORT

Approval of the recommended actions will support the Department of Public Health’s effort to address chronic disease prevention throughout the County. The California Department of Public Health (CDPH) funding authorizes the Department to provide prevention and management support services to local communities with adult populations at high risk for or with a high prevalence of diabetes, with no increase in Net County Cost. This item is countywide.

 

ALTERNATIVE ACTION(S):

 

There are no viable alternative actions. Should your Board not approve the recommended actions, the Department would be unable to complete Prevention Forward intervention services.

 

RETROACTIVE AGREEMENT:

 

The recommended revenue agreement required revisions by CDPH, with the final received on May 6, 2019 and retroactive to April 1, 2019. 

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended actions. The recommended revenue agreement is funded by the CDPH for Prevention Forward interventions, provided by the Centers for Disease Control and Prevention (CDC). The allocation ($43,000) will pay for personnel costs, and program related services to prevent delay or control diabetes. Maximum compensation under the one-year and three-month recommend agreement shall not exceed:

 

                     $20,000 from April 1, 2019 through June 29, 2019

                     $23,000 from June 30, 2018 through June 29, 2020

 

Sufficient appropriations and estimated revenues are included in the Department’s Org 5620 FY 20119-20 Recommended Budget.  The recommended grant award allows for full reimbursement of indirect costs; the current rate for FY 2018-19 is 25.42%.

 

DISCUSSION:

 

The non-competitive Prevention Forward intervention funds support services to local communities with populations at high risk for diabetes. However, since the Department was awarded funding directly from the CDC (NOFO 18DP-1817) to address cardiovascular disease prevention and management, CDPH will allow the recommended funding to be used for activities related to diabetes prevention and management.

 

The strategies specified in the recommended agreement will enable the Department to fulfill the following three objectives:

 

1.                     Assess use of health care reporting systems to identify, report standard clinical quality measures, and/or refer patients with chronic conditions to nationally recognized lifestyle change programs.

2.                     Identify policies and procedures within health care organizations to identify, manage, and prevent chronic conditions.

3.                     Assess use of team-based models to manage, monitor, and refer patients with chronic conditions to nationally recognized lifestyle change programs.

 

The proposed funding will allow the Department to fulfill the following scope of work activities:

 

                     Assist the State in conducting local assessments of use and referral of lifestyle change programs;

                     Inclusion of non-physician team members in the coordination of care

                     Increase recruitment and retention of patients into diabetes prevention and management programs; and, 

                     Educate providers and community members of the availability of diabetes prevention and management programs.

 

The recommended agreement contains two indemnification clauses requiring the County to indemnify the State.  There is a mandatory dispute resolution procedure included in the event there is a dispute between the County and the State. The termination language provides for the State to terminate the agreement immediately for cause and without cause upon 30 days advance written notice.
