RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute an Agreement with Valley Health Team Incorporated, for services to improve prevention and management of heart disease and stroke through clinic-based strategies and community clinical linkages, effective upon execution through September 30, 2023, which includes a two-year three-month base contract, with two optional one-year extensions ($1,399,997).

REPORT

Approval of the recommended actions will allow the Department of Public Health to work with Valley Health Team Incorporated (VHT), a Federally Qualified Health Clinic (FQHC), to provide services that contribute to the reduction of hypertension rates and to increase detection and management of heart disease, with no Net County Cost. This item is countywide.

 

ALTERNATIVE ACTION(S):

 

There is no viable alternative action. Should your Board not approve the recommended action, the Department would be unable to fulfill required cardiovascular disease management and prevention services outlined in the Centers for Disease Control and Prevention (CDC) Cooperative Agreement Grant Award scope of work.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action. The recommended agreement is fully funded with the CDC Cooperative Grant award approved by the Board on February 26, 2019.

 

The maximum compensation is $1,399,997 and funds staffing, services, and supplies to support chronic disease prevention and management during the federal fiscal year funding cycle:

 

                     $199,999.95 in FFY 2019 (execution - September 29, 2019);

                     $299,999.27 in FFY 2020 (Sept. 30, 2019 - Sept. 29, 2020);

                     $299,999.27 in FFY 2021 (Sept. 30, 2020 - Sept. 29, 2021);

                     $299,999.27 in FFY 2022 (Sept. 30, 2021 - Sept. 29, 2022); and

                     $299,999.27 in FFY 2023 (Sept. 30, 2022 - Sept. 29, 2023).

 

DISCUSSION:

 

On September 29, 2018, the Department received the CDC Cooperative Agreement Notice of Award for Category B funding, focused on cardiovascular disease.  Category B funding focuses on improving prevention and management of heart disease and stroke through clinic-based strategies and community clinical linkages.

 

On January 24, 2019, the Internal Services Department - Purchasing Division (ISD - Purchasing) issued Request for Proposals No. 19-044: Collaborating for Wellness Program Services to solicit local qualified health care clinic systems to complete the scope of work activities:

 

                     Identify patients at risk for hypertension;

                     Promote community lifestyle change programs;

                     Increase and refine the referral system using electronic health records (EHR);

                     Expand the prescription (Rx) for Health community referral program;

                     Engage non-physician team members; and,

                     Work with local pharmacists to promote medication therapy management.

 

The RFP was posted on the County’s Public Purchase website, and closed on February 21, 2019, with no proposal submissions.

 

ISD-Purchasing instructed the Department to seek any known vendors who were qualified to provide the required services. The Department contacted with VHT and determined the vendor to be qualified and interested in providing the services. 

 

VHT possesses the necessary organizational expertise, has qualified personnel, has access to patients in the targeted population, and has established community partnerships with the ability to begin Year One work immediately and at full pace.

 

To ensure the program succeeds in its goal to provide services and build systems to improve the health of County residents at high risk for heart disease, VHT will:

 

                     Identify patients that are currently undiagnosed or who have uncontrolled hypertension using EHRs;

                     Expand the Rx for Health to track referrals;

                     Work with various community organizations to increase the number of patients who enroll in CDC approved lifestyle interventions to learn how to control and manage their hypertension; and

                     Engage non-physician team members (clinical staff) to increase patient engagement skills through trainings and by updating internal protocols and policies.

 

With your Board’s approval, the recommended agreement with VHT will enable the Department to fulfill the CDC Cooperative Agreement Award scope of work activities on improving prevention and management of heart disease.
