RECOMMENDED ACTION(S):

TITLE

Approve and authorize the Chairman to execute a retroactive revenue Agreement with Children and Families Commission of Fresno County for the Home Visitation Coordination - Referral Liaison, effective upon execution through June 30, 2022, total not to exceed $150,000.

REPORT

There is No Net County Cost associated with the recommended action.  Approval of the recommended action will provide the Department of Public Health with two years of funding from Children and Families Commission of Fresno County (First 5) to support a Home Visitation Coordination - Referral Liaison.  The First 5 funding will support salary, benefits, and indirect costs for a centralized referral screener position to improve client eligibility and enrollment across Fresno County home visitation programs.  This item is countywide.

 

ALTERNATIVE ACTION(S):

 

There are no viable alternative actions.  Should your Board not approve the recommended action, the Department would not be able to accept First 5 funds, which supports salary, benefits, and indirect costs for a referral screener position.

 

RETROACTIVE AGREEMENT:

 

The recommended agreement is retroactive to October 22, 2020 with services to begin no earlier than when the agreement is fully executed by First 5 and the County.  It was received from First 5 on February 22, 2021 and the time required to prepare and review the recommended agreement did not allow presentation to your Board at an earlier date.

 

FISCAL IMPACT:

 

There is no increase in Net County Cost associated with the recommended action.  The maximum amount of the two-year revenue agreement is $150,000 ($75,000 for each year).  The First 5 funds will help offset salary, benefits, and indirect costs for a public health nurse position currently using Realignment funds to complete duties of the referral liaison.  The recommended revenue agreement allows indirect cost recovery of 14.7165% ($9,621) of total personnel costs; the Department’s indirect cost rate is 22.674%.  The 7.9575% ($5,202) difference will be covered by Health Realignment.

 

DISCUSSION:

 

The First 5 California Home Visitation Coordination (HVC) funding project was designed to help counties create sustainable, unified systems that support families with home visiting services they need and maximize available funding to serve a bigger population.  First 5 Fresno County serves as the lead fiscal agency and subcontracted a portion of the HVC funding to the County to support a centralized referral liaison to improve client eligibility and enrollment workflow across home visitation programs.

 

Approval of the recommended action will allow the Department to accept the First 5 funding that will support a public health nurse (PHN) position to carry out the duties of the HVC referral liaison.  The referral liaison will initially assist the HVC project with mapping, understanding, and building the capacity of the home visitation referral system in the County.  The referral liaison will support the Home Visitation Network, a network of home visitation programs across multiple agencies in Fresno County, in the implementation of the First 5 California HVC Funding Action Plan, develop the process and forms for expanding the triage and referral system, and identify system gaps. Once the initial phase is complete, the referral liaison will be able to receive and route referrals through an established triage system to connect families to home visitation services across multiple Fresno County agencies.

 

The recommended agreement allows for execution also by electronic signatures only in the following formats: faxed versions of an original signature, electronically scanned and transmitted versions of an original signature, or an "e-signed" certified document (e.g. DocuSign).
