RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute an Agreement with Avenal Community Health Center dba, Aria Community Health Center, to promote community health support and to provide health intervention activities for implementation of Fresno County's Advancing Health Literacy Intervention Program, effective May 3, 2022 through June 30, 2023, total not to exceed $500,198.
2. Approve and authorize the Chairman to execute an Agreement with Reedley Community Hospital dba, Adventist Health Reedley, to promote community health support and to provide health intervention activities for implementation of Fresno County's Advancing Health Literacy Intervention Program, effective May 3, 2022 through June 30, 2023, total not to exceed $500,000.
REPORT
There is no additional Net County Cost associated with the recommended actions, both of which will help address the health literacy needs of residents on the west side of the County of Fresno through a variety of outreach and education intervention strategies. These contracted services are part of the newly formed Fresno County Advancing Health Literacy Intervention Program. Funding for this program comes from the Advancing Health Literacy to Enhance Equitable Community Responses to COVID-19 grant received from the United States Department of Health and Human Services, Office of the Assistant Secretary for Health. This pertains to Districts 1 and 4.
