RECOMMENDED ACTION(S):
TITLE
1. Approve and authorize the Chairman to execute revenue Agreement with the California Department of Public Health for a Nutrition Education and Obesity Prevention Grant to provide nutrition education and community engagement services, effective October 1, 2016 through September 30, 2019 ($5,999,043).

2. Approve and authorize the Chairman to execute Amendment II to Agreement No. 13-721 with Public Health Institute, an increase of $80,578, to provide nutrition education and community engagement services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($1,023,623).

3. Approve and authorize the Chairman to execute Amendment II to Agreement No. 13-722 with Fresno County Superintendent of Schools through its Office of Education, an increase of $70,190, to provide nutrition education services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($951,417).

4. Approve and authorize the Chairman to execute Amendment I to Agreement No. 13-723 with California Health Collaborative, an increase of $58,749, to provide nutrition education and community engagement services for an additional three months, effective October 1, 2016, increasing the maximum for the three-year term of November 12, 2013 through December 31, 2016 ($817,068).
